Each year, our community grants provide funds to aid health-focused nonprofit organizations in their mission. Organizations must be invited to participate in the grant process, and are required to provide services in at least one of the following areas: Access to health care for underserved populations; chronic disease management programs and tobacco cessation and prevention services.
