More than half of Americans live with one or more chronic diseases, presenting complex and often costly challenges for both patients and the broader health care system. To help Americans prevent and manage their chronic conditions, CVS Health provides local, personalized touchpoints to care.

Expanding Access to Preventive Care

Effective preventive care helps people avoid or delay disease, keeps conditions from worsening and reduces costs. That is why we’re making it easier for patients to access tools and resources that help prevent chronic conditions from developing.

We offer a dozen health care screening services to identify and monitor risk factors for chronic disease, as well as adult and pediatric vaccinations to protect patients from preventable diseases. We’re also committed to helping patients make healthier choices in their everyday lives. Currently, half of all food options at our retail locations are “better-for-you” choices to help patients make nutritious food selections to promote better health.

Improving Medication Adherence

Patients with chronic conditions often have complex care regimens to follow in between their doctors’ visits. To help these patients, we offer personalized programs that simplify medication adherence. Our adherence programs are making it easier for patients to refill their prescriptions, manage their lifestyle needs and reduce overall health care costs.

Engaging Patients Where They Are

It’s not always easy for patients to get the consistent support they need to manage their care. In fact, a national study revealed that nine in 10 patients (91%) say they need more help with chronic disease management.Kelton on behalf of West Healthcare, February 2017. For example, ScriptPath™ Prescription Schedule and labeling are available in our 9,800 retail pharmacy locations, making medication dosage and scheduling more convenient. We also offer personalized counseling interventions through Pharmacy Advisor in-person or through our digital application, resulting in improved adherence for patients managing chronic diseases. Furthermore, our specialty infusion services through Coram serve 20,000 patients each month in the comfort of their own homes.
