Milken Institute Facilitates Meaningful Discussion on the Social Determinants of Health

Research shows that your environment can be more important than your genetic code when it comes to improving your health. In fact, 60 percent of our life expectancy is determined by social and environmental factors.

To address this issue, Tom Moriarty, chief policy and external affairs officer, and general counsel at CVS Health, recently joined a panel discussion at the Milken Institute Future of Health Summit to discuss how players across the health system can implement solutions that address the personal and financial impact of the social determinants of health, including housing, food and transportation. Moriarty was joined by representatives from the American Public Health Association, BUILD Health Challenge, DC Green and Socially Determined who shared best practices and innovations to tackle the social determinants of health locally.

Below are three key takeaways from the panel discussion.

  • A New Center of Care to Support Patient Needs: Most of our health and well-being happens outside of the doctor’s office – where we live, learn and work. The panel emphasized the importance of understanding the needs of each patient and why local innovations can help address the factors that impact overall health. According to Moriarty, this is where CVS Health can make a difference. Currently, 71 percent of patients live within a five mile radius of a CVS Pharmacy and we utilize our community footprint to expand access to high-quality health services.

  • Improving Local Access to Care is Key: Consider, for example, that 40 percent of physician-ordered lab tests aren’t completed – oftentimes as a result of facilities not having extended hours and the patient lacking access to public transportation to the facilities. According to Moriarty, our MinuteClinic offering can help fill this gap in care. Our extended hours and broad community reach increase access to care. Data shows that up to 50 percent of patients who visit a MinuteClinic don’t have a primary care provider. Furthermore, patients visit on nights and weekends – when other sites of care are closed.

  • Public-Private Partnerships Deliver Value: Panelists agreed that housing and food insecurity have the greatest impact on community health and well-being and additional support is needed for vulnerable patients. To meet these needs, the panel highlighted the value and promise of collaboration across the government, nonprofit and private sectors. Moriarty shared the example of how CVS Health is helping to improve access to safe housing in Philadelphia, Pennsylvania. We recently contributed $4 million to the Inglis Methodist Gardens project to support the development of a four-story, 47-unit apartment building in West Philadelphia – serving a mixed population of long-term care recipients and people at risk for homelessness.

MinuteClinic Launches Video Visit Offering in Oregon and Washington

New service introduces Oregon and Washington residents to the high-quality care offered by MinuteClinic

WOONSOCKET, R.I. CVS Health (NYSE: CVS) today announced that MinuteClinic, the company’s retail medical clinic, has rolled out its video visit offering in the states of Oregon and Washington. People in these states with minor illnesses, minor injuries and skin conditions can now seek care through MinuteClinic Video Visits, a telehealth offering. MinuteClinic Video Visits provide patients with access to care 24 hours a day, seven days a week from their mobile device or computer.

“We’re excited to introduce the states of Oregon and Washington to MinuteClinic through our new MinuteClinic Video Visit offering, as we continue to build our national practice for this virtual care service,” said Sharon Vitti, President, MinuteClinic. “MinuteClinic Video Visits demonstrate our commitment to delivering high-quality care when and where our patients need it, at prices they can afford; providing people who live and work in these states with access to an innovative, on-demand health care option right from their cell phone or computer.”

MinuteClinic Video Visits can be used to provide care for patients ages two years and older who are seeking treatment for a minor illness, minor injury, or a skin condition and are initiated through a computer or mobile device, including through the CVS Pharmacy app. Patients who opt to seek care through a MinuteClinic Video Visit will experience the same high-quality, evidence-based care they receive at traditional MinuteClinic locations inside select CVS Pharmacy and Target stores.

When requesting a video visit, each patient will complete a health questionnaire and be matched to a board-certified health care provider licensed in their state. Prior to the start of the video-enabled visit, the provider will review the completed questionnaire together with the patient’s medical history. During the MinuteClinic Video Visit, the provider will assess the patient’s condition and determine the appropriate course of treatment following evidence-based clinical care guidelines. For patients who require a prescription as part of their treatment plan, the provider will submit the prescription to the patient’s preferred pharmacy. If it is determined the patient should be seen in person for follow-up care or testing, the provider will recommend that the patient visit a health care provider in their community, such as their primary care provider or a nearby MinuteClinic location.

A MinuteClinic Video Visit costs $59, which is currently payable by credit, debit, FSA and HSA cards. Insurance coverage will be added to the experience in the coming months. The service, first introduced in August 2018, is now available in a total of 32 states - Arizona, Arkansas, California, Connecticut, Delaware, Florida, Hawaii, Idaho, Indiana, Kentucky, Louisiana, Maine, Maryland, Michigan, Minnesota, Mississippi, Missouri, Nebraska, New Hampshire, New Mexico, Oklahoma, Oregon, Pennsylvania, Rhode Island, South Carolina, Tennessee, Texas, Utah, Vermont, Virginia, Washington and Wyoming -- and Washington D.C. Since MinuteClinic Video Visits launched in August 2018, thousands of Video Visits have been conducted, with more than a third of them taking place outside of traditional clinic hours.

Information about initiating a MinuteClinic Video Visit is part of the clinic locator on the MinuteClinic web site for states where the service is available. A patient can also initiate a MinuteClinic Video Visit directly from the CVS Pharmacy app.

POLITICO Partnership Elevates Discussion on Social Determinants of Health

Tom Moriarty, chief policy and external affairs officer, and general counsel, recently spoke to 100 health care and policy influencers at a POLITICO Live event in Philadelphia, Pennsylvania, about the importance of local and personalized solutions in addressing the social determinants of health. The event was part of the Health Care Innovators series, sponsored by CVS Health, which showcases leading voices and practices in health care innovation.

Understanding Community Health Care Needs

Most of our health and well-being happens outside of the doctor’s office where we live, learn and work. Furthermore, data show that 60 percent of our life expectancy is determined by factors such as housing, transportation, education and food.

Moriarty noted that these factors underscore why we must understand and analyze how local environments impact health—and the importance of data in the U.S. News & World Report Healthiest Community Rankings. In Philadelphia, nearly one in five residents smoke and more than one-fourth are grappling with obesity. According to Moriarty, our communities are ripe for health care innovation and we have an opportunity to improve health outcomes by creating meaningful touchpoints to care.

Expanding Access to Care Locally

Access remains a key challenge in helping patients manage their conditions. According to Moriarty, community health care access can be defined by two tracks: the availability of primary care and the ability to get to where care is offered.

To demonstrate how CVS Health can address these tracks, Moriarty shared an example of “Diane,” a single mother of two who recently received a diabetes diagnosis. There could be a number of obstacles in her way. First, it may be hard for her to take time off during business hours for appointments. Next, she may face difficulties in getting the testing and labs she needs for diabetes. Research shows 40 percent of physician-ordered lab tests aren’t completed—oftentimes as a result of facilities not having extended hours and the patient lacking access to public transportation to that facility.

According to Moriarty, this is where CVS Health is making a difference. Today, 71 percent of Americans live within five miles of a CVS Pharmacy location. And people come to their pharmacy frequently: whereas a patient with diabetes like “Diane” might only see her physician four to five times a year, she will likely see her pharmacist as many as 18-24 times in the same year.

Moriarty highlighted how we’re utilizing our community footprint to provide timely and targeted interactions with patients like “Diane.” For example:

  • Our MinuteClinic offering is complementary and collaborative to primary care—and helpful to the system overall. We offer treatment for 125 conditions from trusted providers. Furthermore, our extended hours and broad community reach can help address gaps in care.

  • To build on our MinuteClinic services and improve care coordination, we recently piloted HealthHUB—a new, first-of-its-kind concept offering new product categories, digital and on-demand health tools and trusted advice. This concept will be brought to the Philadelphia and Southern New Jersey market in the coming months.

Improving Health Care Affordability

Along with access to care, affordability is a top health care priority for patients. Moriarty emphasized how CVS Health is doing more to help ensure patients get the medications and care they need at the best possible cost.

For example, data show that 40 percent of patients do not pick up their prescriptions when out-of-pocket costs per prescription exceed $200. Moriarty noted that if patients are unable to afford their medications, they get sicker and their care becomes even more expensive. CVS Health has developed solutions to change that.

  • Through our real-time benefits program, we’re providing tools to doctors so they can see what a medicine is going to cost, and recommend lower cost, clinically appropriate options to the patient. More than 100,000 prescribers are using this program—leading to an average of $90 savings per prescription.

  • We’ve also pioneered digital tools, including the Rx Savings Finder, which help our retail pharmacists find patients savings when they do reach the pharmacy counter.

Providing Discounted Care to Special Olympics Athletes at MinuteClinic

Athletes, perhaps more than anyone else, understand the importance of being on top of their physical health.

To make it easier for some of our country’s most impressive athletes to access lower cost, high-quality care, MinuteClinic is working with Special Olympics to provide sports physicals at a discounted rate.

From now through July 31, 2020, Special Olympics athletes will be able to present a voucher at any MinuteClinic location in CVS Pharmacy or Target and receive a sport physical at the discounted rate of $49, no appointment necessary.Local state organizations may have alternate arrangements.

This promotion provides all Special Olympics athletes with access to this necessary care regardless of their insurance status or whether they have a primary care provider.

Easy-to-Access, Lower-Cost Care

Athletes aren’t the only ones who benefit from the type of proactive care that MinuteClinic offers. With 1,100 locations in 33 states and Washington, D.C., patients have access to a wide range of services at MinuteClinic, including wellness screenings, vaccinations, and chronic condition monitoring.

And when a minor illness or injury does arise, MinuteClinic’s nurse practitioners and physician assistants can often provide care for those conditions as well, including earaches, sprains, skin conditions such as rashes or poison ivy, sore throats and infections of the respiratory system or urinary tract.

As we transform the health care experience for all patients, we pay special attention to the needs of patients with chronic diseases who seek care more frequently. We continue to invest in the development of smart and robust solutions to address the health impacts and escalating costs related to chronic disease in the U.S. Our efforts include expanding preventive care and treatment for chronic conditions, improving medication adherence, coordinating care across the healthcare system, engaging patients where they are, and offering healthier product options at our stores.

Personalizing Care Solutions

Our Transform Care programs continue to deliver frequent and highly personalized care to millions of CVS Caremark members who are managing a chronic condition. The programs combine patient-related health and pharmacy data with drug cost management strategies that make care more effective, affordable and easy to navigate.

Transform Diabetes Care, introduced in 2016, has since been adopted by more than 230 CVS Caremark clients, representing 3.4 million members. The program uses advanced analytics to identify members with diabetes and target appropriate interventions. A certified coach develops and helps execute personalized management plans for Type 2 diabetes patients based on their pharmacy data and electronic health record information. The patients use at-home, connected glucometers for glucose readings, which their coach can monitor to offer help as needed. The program also ensures patients receive prescription refill reminders, in-person counseling on diabetes management, and comprehensive diabetes visits at MinuteClinic locations at no out-of-pocket cost.

By 2021, we plan to offer an enhanced diabetes program that combines CVS Health’s local and pharmacy care with Aetna’s care management, targeting, analytics and other capabilities, resulting in a best-in-class diabetes and chronic offering. The enhanced program will derive from the current Transform Diabetes Care model, making for a seamless transition and member experience for clients who wish to adopt the enhanced product. Features of the enhanced product may include:

Aetna’s care managers can provide a single point of contact for members with chronic conditions regardless of which conditions members have. The member’s care manager will also have access to the member’s information from their engagement in the Transform Diabetes Care program and can coach based off of that information.

Creating Comprehensive Programs for Complex Conditions

Another way we are reducing costs and enhancing patient care is through the development of comprehensive programming designed specifically for complex chronic conditions like kidney disease and cancer.

In 2018, we introduced a program to support patients with End Stage Renal Disease (ESRD), a disease that affects nearly 700,000 Americans. Taking a staged approach, our program seeks to slow the progression of ESRD and reduce associated hospitalizations. Beginning with early disease identification and patient education, the program will follow with the development of a comprehensive home dialysis program, for both hemodialysis and peritoneal dialysis. Understanding that longer, more frequent hemodialysis treatments often lead to better outcomes, we will initiate a pivotal clinical trial of a new home hemodialysis device in support of a planned U.S. Food and Drug Administration (FDA) submission to obtain market clearance.

We will select new program participants by determining who is most at risk for kidney disease and progression to kidney failure, as identified through medical and pharmacy claims data and predictive analytics algorithms, in accordance with HIPAA. Our Accordant Care nurses will engage these individuals to educate them on their risk level, important tests they should consider taking, and the need to see a nephrologist. This team will work with individuals as they progress toward ESRD.

We also work with our health care partners and clients to help manage the cost of oncology care while ensuring access to vital therapies for patients. Our approach promotes evidence-based cancer care, lower-cost but clinically equivalent treatment options, and a holistic patient care model that helps patients adhere to therapy. An important component of our cancer care approach is helping providers stay up-to-date with the latest research and advancements, a challenge in the fast-paced field of oncology.

In 2018, we partnered with the leading oncology organization National Comprehensive Cancer Network (NCCN) to provide physicians with a tool to access the latest evidence-supported information. Treatment Guidelines from the NCCN are integrated into Novologix, CVS Health’s proprietary technology platform, and serve as the clinical foundation of multi-drug regimen prior authorization (PA) for physicians and patients. Novologix helps drive cost-effective regimen selection by determining treatment coverage that meets treatment plan needs.

Meeting Women’s Health Care Needs

CVS Health works vigorously to expand access to high-quality, convenient and affordable women’s health care services through our MinuteClinic, retail pharmacies in communities nationwide and online or in the palm of their hand. As part of our focus on transforming the consumer health care experience, we are committed to providing women with access to the coordinated services and care they need to manage conditions and determine what is best for their health.

Providing Convenient Access to High-Quality Women’s Care

Women, who are often key in maintaining healthy families, access the health system more frequently than men, both for themselves and on behalf of their children. In turn, this means that their own health care can take a backseat to the needs of others.

Many women may find it difficult to receive care at a convenient time and location. For example, a survey conducted by Morning Consult and sponsored by CVS Health revealed that 90 percent of women consider the convenience of location important when accessing health services, while 85 percent consider flexible and extended hours important when selecting a health care facility.Morning Consult poll conducted from October 25-26, 2018, among a national sample of 2,201 adults

Our more than 1,100 MinuteClinic locations offer a full range of essential women’s health services. These services include access to birth control, the human papillomavirus (HPV) vaccine, treatment for yeast infections and pregnancy, urinary tract and bladder infection testing. Additionally, MinuteClinic providers also help women better understand their reproductive health options and provide referrals to primary care physicians or OB/GYNs, as needed.

CVS Health also launched a MinuteClinic University Health program, aimed at addressing the health care gap when young adults leave home for school. Our MinuteClinic locations in certain geographic areas near universities and campuses offer additional services for sexual health, women’s health and well-being screenings.

Increasing Patient Access to Birth Control

Ensuring accessible and affordable medications, including birth control, is a priority at CVS Health. For example, CVS Pharmacy patients can pick up prescriptions ordered through the CVS app on their phone, online, in person at CVS retail pharmacies, or they can have their medications delivered through our home delivery program. Additionally, CVS Caremark Members can receive their prescriptions through our mail order pharmacy, and throughout our network of 68,000 retail and mail order pharmacies.

We also work with public health advocates across the country to ensure that women can be obtain a birth control prescription from their local pharmacist. Pharmacists are currently allowed to prescribe oral birth control medications through Prescriptive Authority and Collaborative Practice Agreements in 13 states and the District of Columbia – covering 2,464 CVS Pharmacy locations. And we will be able to do so in 14 more states by the end of the year.

Affordable Access in the Community

Through community partnerships and philanthropies, we are able to address some of the health challenges women face locally. The CVS Health Foundation has partnered with a variety of groups who are on the front lines of improving access to women’s health services.

For example, we’ve established a multi-year partnership with the National Association of Free & Charitable Clinics (NAFC) with grants awarded to free and charitable clinics to increase access to care, improve health outcomes and lower overall health care costs. Today, 58 percent of NAFC patients are women – and they rely on 1,400 clinics nationwide for services including breast and cervical cancer screening, maternal and infant health and much more.https://www.nafcclinics.org/sites/default/files/NAFC%202019%20Infographic.pdf Since launching the partnership, the CVS Health Foundation has delivered a total of more than $4.5 million in funding to free and charitable clinics across the country.

Addressing Women’s Heart Health

Cardiovascular disease remains the number one killer among women and accounts for one-third of deaths annually.https://www.heart.org/-/media/data-import/downloadables/heart-disease-and-stroke-statistics-2018---at-a-glance-ucm_498848.pdf To address the widespread impact of this disease, we help women understand their risk for heart disease and empower them to take the next best action for their health. Throughout February 2019, we provided preventive heart health screenings at no cost in which women learned their five key personal health numbers that help determine their risk for heart disease: total cholesterol, HDL (good) cholesterol, blood pressure, blood sugar and body mass index.

Additionally, our support of the American Health Association Go Red for Women campaign, includes a new three-year, $15 million commitment to life-saving cardiovascular research and education.

Heart Health with a Haircut: Bringing Care to Underserved Communities

A recent heart health community event in Las Vegas highlighted our ability to reach underserved communities, diagnosing and treating people who may not typically have easy access to health care services.

The event in partnership with U.S. Rep. Steven Horsford’s office was held outside a local African American barbershop as a way to bring needed health care to an area with few providers.

“Barbershops and salons provide safe places to have honest conversations about our lives, our health and our community,” said Horsford, noting the important role these spaces play as neighborhood gathering points. “The health care shortage in Nevada is very hard on the African American and Hispanic communities, and we need more companies like CVS Health to step into that gap.”

Health disparities continue to be an issue affecting minority communities, including Nevada. In 2017, the African American community in Nevada had the highest mortality rates of heart disease (291.7 per 100,000 people). The day’s activities showcased how CVS Health and Aetna are bringing together the combined power of a health care provider and insurer to help consumers achieve improved health. In addition to the free health screenings, including blood pressure, cholesterol, weight and glucose monitoring, a local MinuteClinic practitioner was on site to help patients navigate their health concerns.

“Our vision as a combined company is to help people achieve their unique health ambitions,” said Daniel Knecht, M.D., Vice President of Clinical Strategy and Innovation for CVS Health. “This kind of hyper-local care model allows us to step into the community, and successfully engage with the people we serve in a culturally appropriate and personalized manner.”

Screening results reinforced the need for such care models, especially in underserved communities like West Las Vegas. Recent studies have demonstrated the key role barbershops play not only in screening but connecting patients to ongoing treatment. In April 2018, a landmark trial published in the New England Journal of Medicine found significant, sustained reductions in blood pressure among patients who engaged with a pharmacist in the barbershop.https://www.nejm.org/doi/full/10.1056/NEJMoa1717250 CVS Health is working closely with these national experts to bring the model to more communities.

Horsford, who had coronary artery bypass surgery at the age of 40, understands the importance of hypertension management and living a healthy lifestyle. He noted that events like the heart health clinic can extend health care services to where they’re needed most.

“In the African American community, we don’t talk enough or share enough between the generations. We need to break the cycle of health care disparities,” he said. “I appreciate CVS Health’s desire to bring their services into the community.”

An afternoon panel discussion moderated by Horsford and featuring CVS Health leaders engaged local community leaders and providers in a conversation about the special health care needs of underserved communities and potential solutions.

David Casey, Chief Diversity Officer for CVS Health, noted that the U.S. health care system needs to do a better job “figuring out how to bring personalized health care to consumers not just where they live, but how they live.”

He pointed to CVS Health initiatives such as Building Healthier Communities, a five-year, $100-million commitment to support critical partnerships with nonprofit organizations, as an example of effective public-private efforts. Another new initiative called Destination: Health will help some of Aetna's most vulnerable Medicaid and Dual eligible members access social services within their community more easily.

Las Vegas boasts nine MinuteClinics, which provide walk-in services seven days a week. Nurse practitioners and physician assistants offer expert medical care for a wide array of minor illnesses, minor injuries and skin conditions as well as monitoring of chronic conditions and the provision of immunizations and physicals.

Expanding MinuteClinic Care to Veterans Nationwide

Support for our country’s veterans and members of the military is central to the work we do at CVS Health, whether it’s through our Workforce programs or our efforts to connect this population with easily accessible, high-quality care.

Since 2016, through a partnership with the U.S. Department of Veterans Affairs (VA), CVS Health has provided health care services to more than 4,000 veterans at select MinuteClinic locations.

Now, through the enactment of the VA MISSION Act, we have been able to expand that care to all of our 1,100 MinuteClinic locations nationwide.

“Our participation in the MISSION Act will give deserving veterans across the country greater access to quality health care when and where they need it while ensuring they are still linked into the Veterans Health Administration,” said Sharon Vitti, President of MinuteClinic. “We’re proud to be able to support the VA on such a meaningful and impactful program.”

Care for Veterans through the MISSION Act

Under the MISSION Act, veterans can now access urgent care and walk-in medical services under their VA benefits without pre-authorization. Veterans who meet certain eligibility criteria can receive care at any MinuteClinic location, including treatment for minor illnesses, minor injuries and skin conditions. Following the visit, MinuteClinic is able to make the full record of medical services provided available to the VA with the patient’s consent.

The expansion builds upon our previous pilot programs with the VA, which gave veterans the option to receive care at MinuteClinic locations in Corpus Christi, Texas; Palo Alto and Santa Clarita, California; and Phoenix, Arizona.

Our History of Support for Veterans

Our support of the MISSION Act is one example of the many ways we stand with veterans, active military and their families.

We have several Workforce Initiatives programs dedicated to recruiting, hiring, training and retaining veterans to build a pipeline of workforce talent. Since 2015, CVS Health has hired almost 15,000 people with military experience and more than 5,000 military spouses.

In addition, we provide charitable support to military and veteran-focused organizations, including the National Guard, Operation Reinvent and the USO. And colleagues can connect through the CVS Health Colleague Resource Group BRAVE, which is comprised of nearly 1,400 members with a passion to serve those who have served our country.

We were recently recognized for our efforts to support veterans and military members.

In May, Military Times named CVS Health to its 2019 list of best companies for veterans seeking a civilian job.

And four CVS Health managers were recently honored with 2019 Patriot Awards from the Rhode Island Employer Support of the Guard and Reserve (RI ESGR) for their support of three colleagues serving in the National Guard and Reserve.

Five Things to Know: Getting Vaccinated at CVS Pharmacy

Across the country, in nearly 10,000 communities nationwide, our customers turn to their local CVS Pharmacy to help lead them on a path to better health, not only through our prescription services, but our innovative assortment of health-focused products and services as well.

One of those services is vaccinations. Administered in all of our retail locations nationwide by a certified immunizer and by a nurse practitioner at our MinuteClinic locations, our vaccination offerings go well beyond the annual influenza shot. And since no appointment for vaccinations is required, CVS Pharmacy is a convenient and reliable place for customers to get CDC-recommended vaccines when and where they need them.

Here are some things to know before heading to a local CVS Pharmacy for a vaccination.

1. We offer more than just flu shots.

While vaccine availability may vary by state, all of our CVS Pharmacy and MinuteClinic locations offer most of the vaccinations commonly recommended by the CDC, including pneumonia, MMR (measles, mumps, and rubella), meningitis, and Hepatitis A and B.

Additionally, your pharmacist is an excellent resource for information about what vaccinations are appropriate for your age group.

An appointment for a vaccination is not required, but we do recommend that calling the store first to confirm that the vaccination you’re looking for is in stock.

2. Customers of most age groups can receive vaccinations in our stores.

At CVS Pharmacy, vaccinations are available for adolescents, adults and seniors, while at MinuteClinic, children as young as 18 months can receive certain vaccinations. Call your nearest location to talk to the pharmacist about what vaccinations are right for your age group and what’s available at that location.

3. Talk to your Pharmacist or Nurse Practitioner prior to your vaccination.

Before administering your vaccination, the immunizing pharmacist or nurse practitioner will want to know about any medical conditions you might have, any previous reactions to vaccines, and your medication history if you fill your prescriptions at another pharmacy. They will also be able to advise on any side effects that may result from your vaccination.

4. Most vaccinations are covered by prescription insurance.

In many instances, your vaccination will be covered by either your pharmacy benefit manager or your health insurer, depending on your plan and whether you go to a CVS Pharmacy or MinuteClinic for your shot.

If you’re paying out of pocket, your pharmacist can provide pricing information.

5. Your vaccination can be added to your medical record.

Following your vaccination, with your permission, CVS Pharmacy or the MinuteClinic practitioner can notify your primary care physician so that the shot is added to your medical record.
